Transitioning from Birth t0 Three Program
Welcome to MCIU Preschool! It is our intent to ensure a smooth transition and entry to Preschool Early Intervention services. Parent/caregivers and teachers, in order to obtain copies of our referral forms, please contact us at 484-685-1856.
Please use the button below to access the Birth to Three Interventionist Transition Questionnaire and the Teacher/Caregiver Questionnaire for the Preschool/Daycare teacher to complete.
**If your child is not transitioning from the Birth to Three program and you are new to services/referral, please refer to the “Brand new to services/referral” tab and NOT the above forms.**

A parent or legal guardian who is concerned about their child’s educational development should complete the following form to receive a referral packet from the Montgomery County Intermediate Unit.
The Pennsylvania Department of Education is responsible for law for providing early education services to eligible children age three (3) to school age in the Commonwealth. The Montgomery County Intermediate Unit 23, through an agreement with the Department of Education, is contracted to deliver those services in Montgomery County.
New to PA/Montgomery county
If you are moving to Pennsylvania from another state or you are moving to Montgomery County from another county in Pennsylvania and your child has an active IEP, please do the following:
A parent or legal guardian who is concerned about their child’s educational development should fill out this contact form to receive a referral packet.
Once you have received your packet and are gathering your documents, it is very important to include a copy of your child’s current Educational Plan, Evaluation Report and any other educational or medical documents. In addition to these documents, please make sure you sign a Release of Records form with your previous school district/intermediate unit in order for us to facilitate communication.
Once the forms are completed and returned to the MCIU, you will be contacted by an Early Intervention staff member.

In order to become eligible for Early Intervention preschool services, children of this age group must exhibit 1) a developmental delay of 25 percent or a qualifying disability and 2) require specially designed instruction.
